For many people, these are the activities that define both their work and their home life.
The condition is often underestimated because it begins as a minor nuisance. By the time people seek help, it has often been disrupting daily tasks for months. Understanding what tennis elbow actually involves — and why passive rest is rarely the answer — changes how people approach recovery.

The clinical understanding of tennis elbow has changed significantly over the past two decades. What was once treated as an inflammatory condition is now understood primarily as a tendon loading problem — which changes the approach to treatment entirely.
What Tennis Elbow Actually Is
Tennis elbow — clinically known as lateral elbow tendinopathy — is a condition affecting the extensor tendons that attach at the outer elbow. It is not primarily an inflammatory condition, despite the traditional name "epicondylitis." The current understanding is that it involves a degenerative change in tendon structure, which is why anti-inflammatory approaches often provide only temporary relief. The tendon needs to be progressively loaded and rehabilitated, not just rested.
Who Gets It and Why
Despite the name, most people who develop tennis elbow have never played tennis. It is common in tradespeople, office workers, musicians, surgeons, chefs, parents of young children, and anyone whose daily life involves sustained gripping, repetitive wrist extension, or carrying loads with an outstretched arm. The common thread is sustained demand on the extensor tendons — not sport specifically.
Why It Can Be Slow to Resolve
Tendons have a limited blood supply compared to muscle and bone, which means they receive fewer of the resources that support healing. They also do not rest between uses the way a broken bone can be immobilised. Every time you grip a cup, type a sentence, or open a door, the extensor tendons at the elbow are working. This is why tennis elbow can persist for months, and why passive approaches like rest, ice, and anti-inflammatories often fail to resolve it.
The Role of Load
The evidence base for tennis elbow management has shifted significantly in recent years. We now understand that progressive loading of the tendon — carefully dosed, gradually increased — is more effective than rest for long-term recovery. The goal is not to avoid using the tendon; it is to load it in a way that promotes healthy tissue remodelling. This is the principle behind modern physiotherapy for lateral elbow tendinopathy.
Where Tennis Elbow Shows Up in Daily Life
The condition rarely presents as elbow pain during rest. It presents during gripping and carrying — which means it interrupts the ordinary tasks of daily life in ways that can be surprisingly limiting. Understanding exactly where the impact falls helps identify what recovery needs to address.
The Grip Problem
The most consistent feature of tennis elbow is pain with gripping — and gripping is involved in almost everything done with the hand. Turning a door handle, picking up a kettle, carrying shopping, shaking hands, writing by hand, using tools at work. People with tennis elbow often develop compensatory patterns — shifting tasks to the other hand, avoiding certain movements — that provide short-term relief but can prolong recovery.

Tradespeople — carpenters, plumbers, electricians, mechanics — are among the most commonly affected. Power tools, sustained gripping of hand tools, repetitive wrist motions, and carrying loads all load the extensor tendons. Return-to-trade-work after tennis elbow requires specific preparation for those demands, not just resolution of pain at rest.
Carrying Children and Domestic Tasks
Parents of young children frequently develop tennis elbow from lifting and carrying — particularly with the arm extended and the palm downward (the position that most loads the extensor tendons). The domestic demands that follow a baby — bathing, nappy changing, lifting in and out of prams — are continuous and difficult to modify. Management in this context needs to be practical, not just clinically ideal.
What Recovery From Tennis Elbow Actually Involves
Most people with tennis elbow have tried resting it. For some, rest provides temporary relief. For the majority, symptoms return when activity resumes — because the tendon has not been rehabilitated, only rested. Modern physiotherapy approaches this differently.
Graded Tendon Loading
The evidence for progressive loading in tendinopathy management is now strong. Starting with isometric exercises (contracting without movement), then isotonic loading (moving through range), then progressing to functional activities — this sequence promotes tendon remodelling without overloading tissue that is still recovering. The specific progression depends on the person's demands and how the tendon is responding.
Addressing the Contributing Factors
Tennis elbow rarely develops without a reason. A workstation that forces sustained wrist extension. A tool that requires more grip force than it should. A sudden increase in activity that the tendon was not prepared for. Effective management addresses these contributing factors — not just the symptoms they have produced. Otherwise, the same condition tends to recur when the same demands return.
Managing Activity During Recovery
Complete avoidance of gripping and carrying is not realistic or necessary for most people. The goal is to maintain as much normal function as possible while managing the load on the tendon. Practical strategies — modified grip techniques, task sequencing, tool selection, activity pacing — allow most people to continue working and managing daily life while rehabilitation progresses.
What to Expect in Terms of Timeline
Tennis elbow tends to resolve more slowly than most soft tissue injuries, and the research reflects this — with some studies suggesting typical recovery of 6–12 months in untreated presentations, and considerably faster resolution with appropriate physiotherapy. The variability in timelines is not random; it correlates with how long the condition was present before treatment began and how consistently the loading programme was followed.
Recovery Looks Different at Different Stages
In the acute phase, when pain with gripping is significant, the priority is modifying load while maintaining some activity. Complete rest can actually be counterproductive — the tendon becomes less tolerant of load, making resumption of activity harder, not easier.
In the rehabilitation phase, the priority shifts to building the tendon's capacity to handle the demands of daily life. This is often the stage that people rush through or abandon when pain settles — and it is the stage most responsible for recurrence.
In the return-to-full-activity phase, the focus is on progressively reloading the tendon to the full demands of work, sport, or whatever activity was most affected. This stage should not begin until the earlier stages are genuinely consolidated — and it should not be skipped just because the elbow feels better in daily life.
